Responsibilities
- Identify patient‑specific drug related problem(s) and collaborate with other healthcare providers in providing recommendations and effective solutions in optimising pharmacotherapy.
- Facilitate safe and appropriate use of drugs.
- Perform medication reconciliation for all new admissions in a timely manner as well as regular medication reviews for existing patients.
- Perform bedside medication counselling for patients on discharge to home and ensuring medication list is promptly updated within the day of discharge.
- Ensure in‑house clinical guidelines are regularly updated and adhered to by healthcare providers.
- Communicate effectively when providing professional advice and evidence‑based drug information to patients and the healthcare team.
- Participate in ward rounds.
- Implement and/or review policies, practices, and regulations.
- Oversee pharmacy daily operations.
- Accountable for the procurement, inventory control, distribution, and supply of controlled drugs.
- Ensure twice yearly medicine management audit is conducted for and homecare.
- Supervise and mentor pharmacy technicians in their daily duties.
- Regular review of the collaborative prescribing services.
- Undertake to keep oneself well‑informed of the updated industry guidelines and develop new skills that are necessary for optimal functioning in the job.
Requirements
- Possess a Bachelor Degree in Pharmacy from an accredited college of Pharmacy.
- Registered with the Singapore Pharmacy Council.
- At least 3 years of relevant experience in a healthcare environment preferably.
- Bilingual and ability to communicate in local dialects preferred.
- Proven ability to manage pharmacy operations and supervision of pharmacy technicians.
- Passion to serve and provide good patient care.
- Good interpersonal skills and able to work independently.
- Flexible, adaptable and able to work effectively in varied setting.
- Possess good clinical knowledge.
