Job summary

A leading mental health institution in Singapore is seeking a Case Manager for the Early Psychosis Intervention Programme (EPIP). In this role, you will provide integrated case management and therapeutic services to empower individuals aged 12-40 suffering from early psychosis. Responsibilities include coordinating care, conducting assessments, and developing psychosocial intervention plans with a passionate multidisciplinary team. The position requires a degree in Social Work, Counselling, or Psychology, with a minimum of 5 years of relevant experience.

Qualifications

  • Experience in helping profession with young persons or individuals suffering from psychiatric disorders.
  • At least 5 years of relevant working experience for senior case manager position.

Responsibilities

  • Provide and coordinate continuity of inpatient and outpatient care.
  • Conduct assessments of patients' needs across various domains.
  • Formulate and develop psychosocial intervention plans.
  • Implement intervention plans including counselling and psychoeducation.
  • Review and evaluate effectiveness of intervention plans.

Job Description

Join us, the “Early Psychosis Intervention Programme” (EPIP), to provide integrated case management with therapeutic services in combining direct clinical care and coordinated support across multiple contexts. Working within a multidisciplinary team, you will empower individuals (aged 12 – 40 years old) suffering from early psychosis or at risk of developing psychosis, and their families to take ownership of their recovery journey through evidence-based interventions and interdisciplinary care.

Clinical
  • Provide and coordinate continuity of inpatient and outpatient care from initial consultation through to recovery
  • Conduct assessment of patients for medical, psychological, social, environmental, vocational and financial needs
  • Formulate and develop psychosocial intervention plans with patients and/or caregivers
  • Implement intervention plans, including counselling and psychoeducation to patients and caregivers
  • Review and evaluate effectiveness of intervention plan at appropriate juncture with patients and treating team
  • Manage crises to facilitate safety of the vulnerable
  • Work in close collaboration with multi-disciplinary team and community partners
  • Conduct group work with patients and/or caregivers
Research
  • Conduct research ratings with patients and/or caregivers
Requirements
  • Degree in Social Work, Counselling or Psychology
  • Working experience or internship in helping profession with young persons, or with individuals suffering from psychiatric disorders, will be of added advantage.
  • Must be a team player with passion in mental health recovery, with willingness to learn, and strong communication skills
  • At least 5 years of relevant working experience for senior case manager position
