Job Description:

The nurse works closely with doctors to provide appropriate, comprehensive, and seamless care for patients with chronic conditions. Your role involves:

Nursing Care

• Assess, plan and deliver evidence-based nursing care for chronic conditions patients
• Develop person‑centred care plans addressing bio‑psychosocial needs
• Document care accurately and administer medications safely per protocols

Patient Education & Self‑Management

• Teach chronic disease self‑management (diet, medication adherence, exercise, monitoring techniques)
• Conduct depression, anxiety and functional assessments
• Provide health education and support behaviour change
• Participate in multidisciplinary team discussions for complex cases
• Navigate care systems and coordinate with community partners for continuity
• Monitor patient progression, track outcomes and recall defaulters
• Support chronic care initiatives and health education programmes
• Assist in developing clinical protocols and patient education materials

Care Coordination & Collaboration

• Participate in multidisciplinary team discussions for complex cases
