Position Title: Physical Therapist

The Physical Therapist assesses, plans, organizes, and participates in rehabilitative programs aimed at improving mobility, relieving pain, increasing strength, and improving or correcting disabling conditions resulting from disease or injury. We offer both outpatient and home health settings, providing flexibility for various career preferences. Responsibilities
- Develop, implement, and adjust a rehabilitation plan of care in collaboration with clients, caregivers, and physicians.
- Conduct physical strength tests and screenings to assist in evaluating the patient's level of function.
- Educate and instruct clients, family members, and caregivers on exercise programs and therapeutic procedures.
- Accurately document observations, interventions, and evaluations regarding client care and services provided.
- Provide direct patient care as prescribed, including evaluation and treatment planning.
- Review physician referrals and patient medical records to determine diagnosis and therapy treatment required.
- Record prognosis, treatment responses, and progress in patient charts and databases.
- Identify and document goals, anticipated progress, and plans for reevaluation.
- Assess the effects of treatment and adjust therapies for maximum benefit.
- Participate in educational activities, clinical studies, and professional development.
- Perform additional duties as required.
Qualifications
- Graduation from an accredited Physical Therapy program.
- Current and valid Physical Therapy license (state-specific).
- Previous home health experience preferred but not required.
- Valid Driver’s License, Car Insurance, and Car Registration.
- Strong clinical assessment, organizational, and communication skills.
- Ability to manage multiple assignments and work in a fast-paced environment.
- Proficient in using electronic devices and software for documentation and timekeeping.
- Bilingual (Spanish) is a plus.
- Strong interpersonal skills and sound judgment.
- Ability to work well under pressure and maintain composure in emergency situations.
- Self-motivated with a desire to continuously learn and adapt to medical advancements.
Physical Demands and Work Environment
Ability to talk, hear, and use hands for various tasks. Occasional lifting of up to 50 pounds. Regular standing, walking, sitting, and reaching. Vision abilities required include close and distance vision and focus adjustment. Low to moderate noise level in the work environment.
