What you will do: The Hospice RN Case Manager is a professional nurse responsible for coordinating and directing hospice patient care in various environments (private residence, skilled nursing facility, assisted living, hospital, etc.) based on individual needs. The role involves independent management of a caseload, utilizing advanced assessment, teaching, and decision-making skills. Responsibilities include evaluating patient appropriateness for hospice care, developing care plans collaboratively, providing follow-up assessments, and ensuring documentation complies with agency standards. The RN participates in team meetings, performs on-call duties, and handles holiday staffing as required.

Required Education: Graduate from an accredited Diploma of Nursing Program or Associate’s Degree in Nursing.

  • Current American Heart Association CPR certification

Preferred Licensure/Registration/Certification:

  • Hospice and Palliative Care certification
  • Wound Care certification
  • Additional certifications or education in specialty areas

Required Experience: Minimum of two years in a healthcare setting within the relevant discipline.

Preferred Experience:

  • Medical-Surgical experience
  • Case Management experience
  • Supervisory roles in healthcare
  • Home Care or Hospice experience
