BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a per diem opportunity performing home health visits for adult and geriatric clients throughout Iredell and Rowan Counties.

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, ambulation patterns, strength and endurance, balance testing, and safety assessments.
3. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
4. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
5. Assess and revise the care plan continually, collaborating with nursing in the multidisciplinary team as appropriate.
6. Educate clients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using a touch pad tablet.

1. A current NC Physical Therapist - PT license.
2. At least one year of recent work experience as a Physical Therapist - PT.
3. Graduation from an APTA-approved program or the Committee on Allied Health Education and Accreditation of the American Medical Association, evidenced by transcript or diploma.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Solid computer skills; experience with electronic medical records (EMR) preferred.
