Job description
Job summary

Deliver, in conjunction with the Principal Pharmacist and Head of Pharmacy, a clinical pharmacy service to Bassetlaw LMHT within the Adult Mental Health care unit of the Mental Health Care Group. Work as a pharmacist independent prescriber and hold a caseload of patients who are under the care of the LMHT and who are judged as well and stable enough for discharge from Medical Outpatients. Undertake a patient‑facing clinical role to deliver a high quality and cost‑effective clinical pharmacy service in line with national and local medicines optimisation strategy. A full UK driving licence and vehicle for business use is required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Responsibilities
  • Provide pharmaceutical input at multidisciplinary team reviews, take drug histories and devise patient‑specific pharmaceutical care plans.
  • Provide training sessions for service users and carers on medication and specialist medicines information and advice to healthcare staff, service users and carers.
  • Contribute to initiatives, including the development of treatment and drug usage guidelines that will provide positive clinical and financial outcomes.
  • Ensure that local and national guidelines, policies and protocols are applied at service level via the clinical pharmacy team.
  • Liaise with individual prescribers regarding drug‑prescribing issues within their specialty to ensure maximum benefit in patient treatment, in line with local and national guidelines.
  • Participate in the development and implementation of new ways of working and extended roles for pharmacists and pharmacy staff.
  • Participate in late duty, weekend and Bank Holiday working in accordance with departmental rotas to fulfil contractual obligations and support the on‑call pharmacy service.
  • Undertake all scheduled mandatory training, including information governance, required for the role.
  • Act as a line manager for a small number of pharmacists and/or pharmacy technicians working in LMHTs.
  • Provide professional support to the dispensary on a rota basis, including validation of new prescriptions, checking of dispensing operations performed by pharmacy staff and undertaking occasional dispensing tasks as necessary.
  • When necessary, deputise for the Principal Pharmacist and Head of Pharmacy on appropriate multidisciplinary committees, working groups and in general operational issues.

Additional Information

Applicants will be required to pay for their DBS check. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), which will be deducted from salary over the first 2 months of employment. Applicants are encouraged to enroll for the DBS Update Service; an annual fee of £16 per year applies.
