Job summary

A healthcare provider in North Manchester is seeking a Trainee Assistant Practitioner to support the Intermediate Care Therapy team. In this role, you will deliver patient-centered rehabilitation interventions in the community following hospital discharge. The ideal candidate will have a Foundation Degree in Health and Social Care and preferably prior experience in rehabilitation. A full UK driver's license and access to a vehicle are essential. This position offers ongoing professional development and the opportunity to work with a multidisciplinary team.

Qualifications

  • Foundation Degree in Health and Social Care (Assistant Practitioner) completed or in progress.
  • Experience in a rehabilitation setting is preferred.
  • Valid UK driver's license with access to a vehicle.

Job description
Role Overview

The post holder will work as an integral part of the Intermediate Care Therapy team to provide a high quality patient centred approach to adults in the community in North Manchester.

Responsibilities
  • Rehabilitate and improve function of patients by providing interventions in their own home following acute hospital discharge or illness/ injury at home.
  • Manage own caseload of patients in the community after completing appropriate competencies.
  • Deliver team and office based duties to ensure effective service delivery.
  • Close working with student therapists and other multidisciplinary team members.
Qualifications
  • Foundation Degree in Health and Social Care (Assistant Practitioner) already in receipt or enrolled and working towards this.
  • On successful completion of the 2-year course you will move into a band 4 position.
  • Previous experience within a rehabilitation setting preferred.
  • Competent in IT systems.
  • A full UK drivers licence and access to a vehicle essential to commute between patient visits.
