Overview

We are excited to launch the new Assertive Outreach Team covering North and South of Cambridgeshire and Peterborough. The team collaborates with existing CPFT services to support patients with the most complex and severe mental health needs through a small caseload of intensive community‑based care.

Responsibilities
  • Deliver care coordination to a caseload of patients using the DIALOGUE+ framework; complete assessments and nursing care plans for each patient.
  • Plan, implement, and review care regularly in collaboration with a multidisciplinary team.
  • Maintain high professional standards, leading staff and ensuring smooth day‑to‑day operations of a highly complex service.
  • Provide specialist consultation to the multidisciplinary team, clients, families, and statutory and non‑statutory agencies.
  • Prepare and submit accurate reports for specialist placements, Mental Health Act tribunals, and managers' hearings.
Qualifications
  • Experienced clinical nurse with an interest in innovative and developing services.
  • Full UK driving licence and reliable vehicle ownership.
  • Ability to travel independently around the county meeting strict time deadlines.
  • Strong communication skills and proven leadership within a multi‑disciplinary team.
