Overview

We have a fantastic opportunity for a Team lead to cover maternity leave for 12 months as a Band 7 Team Lead. This role is within Community Mental Health services as we are developing our neighbourhood teams to test new ways of providing mental health interventions for adults with severe mental illness and complex needs.

As part of the neighbourhood team development, services are exploring options to deliver services over 7 days. The Team Lead will manage and take overall responsibility for care quality, performance and clinical activity for the community team within Mental Health for Older People (MHSOP). This is a fantastic opportunity for anyone with a wealth of experience in nursing and management who wants to help make a significant change in the community to improve quality of life and reduce unplanned hospital admissions.

Responsibilities

• Provide professional nursing leadership and guidance, clinical advice and management in relation to the key dimensions of health care quality: patient safety; the effectiveness of all health care interventions; patient experience; and the environment in which care is delivered.
• Take overall responsibility for care quality, performance and clinical activity for the community team within MHSOP.
• Link people to health and social services available in the locality, including community and voluntary sector services, to help remove barriers to access.
• Work with the wider multi-disciplinary team to provide a comprehensive, efficient and effective support service guided by an agreed work plan.
