Job summary

A local authority council in Tamworth seeks a compassionate individual for a 12-month fixed-term role supporting vulnerable adults with learning disabilities and/or Autism. Job description

This is a 12 month fixed term contract to cover maternity leave We are looking for someone to work within the local community with individuals to identify future goals, wishes and aspirations. To work with vulnerable adults who have learning disabilities and/or Autism, helping the person to achieve personal outcomes, ambitions and maximise opportunities, taking control of their lives and making their own choices. Enabling the person to grow relationships and build networks in their local community. To work creatively and proactively to ensure person centred approaches are used for people with disabilities. To work under the managers guidance to follow support plans. Utilise the public transport system to access the local community and public services. To lone work in the community with individuals you are supporting. To maintain electronic contact records of your support and reporting to your line manage any concerns
