Job description

Support Coordinator

We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join the Life After Stroke Service based across North, Mid, and Central Hampshire.

This is an exciting opportunity to work with stroke survivors and their families to support them following a stroke.

Position: S11304 Stroke Support Coordinator

Location: Home-based, Hampshire, specifically North, Mid, and Central Hampshire (Basingstoke/Winchester area). Frequent travel will be required (to include team meetings and other work-related meetings).

Salary: Circa £18,812 per annum (FTE circa £27,400 per annum)

Hours: Part-time, 24 hours per week

Contract: Fixed-term until 31 March 2026, with current funding until that date.

The Role

The service aims to identify and provide key worker support to meet the needs of stroke survivors and carers across the stroke pathway, offering innovative support solutions to help them achieve their desired outcomes.

The Stroke Support Coordinator will:

  1. Support new stroke survivors and their carers from hospital discharge into the community.
  2. Provide personalised information, advice, and support.
  3. Assist stroke survivors in making informed lifestyle changes to prevent further strokes.
  4. Collaborate with NHS colleagues and community services to improve the lives of those affected by stroke.

About You

The ideal candidate will:

  1. Support new stroke survivors and their carers from hospital discharge into the community.
  2. Provide personalised information, advice, and support.
  3. Support stroke survivors in making informed lifestyle changes to prevent further strokes.
  4. Work collaboratively with NHS colleagues and community partners.

This role involves extensive travel across a large geographical area to visit people at home and in community settings. Candidates must demonstrate how they can meet this requirement.
