Qualifications

  • Conduct clinical risk assessments for clients.

  • Oversee admission, treatment, and discharge processes.
  • Develop care plans as a named nurse.

Overview

An exciting opportunity to join our new older adults ward, specialising in care for older adults experiencing mental health conditions including dementia. We provide 24 hour care within a purpose‑built assessment and therapy unit, working within a multidisciplinary approach that optimises individual recovery. We are an activity‑driven, multidisciplinary team with a focus on person‑centred care that supports both clients and their carers.

Responsibilities
  • Be responsible for further assessment, planning and implementation of care using a MDT approach.
  • Conduct appropriate clinical risk assessments of clients within our care.
  • Provide effective advice, guidance and involvement in the decision‑making process alongside clients, culminating in the development and overview of care planning with MDT support.
  • Oversee admission, assessment, treatment and discharge for individuals within our care.
  • Be responsible for the care and assessment of individuals as a named nurse and develop a care plan within this role.
Skills and Qualifications
  • Excellent communication skills.
  • Ability to work to excellent standards both independently and as part of the team.
  • Passionate about working with older adults.
  • Excellent knowledge of the MHA/MCA.
