About the Role

Specialty Doctor in Palliative Medicine – community based within the University Hospitals Coventry and Warwickshire Community Palliative Care team.

The successful candidate will work 3 days a week with the team, providing specialist palliative medicine assessment and advice independently and supporting colleagues across the Coventry Community Specialist Palliative Care Team, Rapid Response, Palliative Support Team and Integrated Neighbourhood Teams. This role requires domiciliary visits; a full UK driving licence and access to a car is mandatory.

Responsibilities
  • Work with Consultants in Palliative Medicine to deliver high quality palliative care.
  • Undertake medical assessment, diagnosis and management of patients in the community.
  • Initiate appropriate medical care and prescribe in line with policies and guidelines.
  • Collaborate with the MDT to initiate appropriate investigations and treatment.
  • Provide telephone or remote support to the team or other health care professionals.
  • Communicate supportive and clear information to patients and their carers regarding their condition and its management.
  • Record clinical notes accurately and in a timely manner.
  • Participate in CPD, appraisal and revalidation.
