Overview

The Specialist Health Visitor will offer timely interventions, intensive support and assessments as early as 13 weeks of pregnancy. This intervention has been proven to not only reduce the number of children becoming looked after but also to reduce health inequalities.

Responsibilities
  • Take responsibility for managing a caseload of individuals/families with complex needs.
  • Use negotiation skills to develop individual Care & Support Plans with individuals/families; continually assessing and evaluating care needs.
  • Analyse own practice, identify new ways of working and implement change.
  • Undertake audit of practice, identify excellence and areas of poor performance, and be able to design programmes to meet any required needs identified.
Qualifications

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
