Specialist Community Mental Health Nurse PCMHN

Are you passionate about supporting adults to have good mental health? Are you a recovery focused and creative mental health nurse whowants to work in a supportive multi-disciplinary team? An opportunity has become available to work within the Goole Cygnet Primary Care Mental Health Network team as aBand 6 Specialist Community Mental Health Nurse.

As part of the role you will need the means to effectively transport yourself between multiple addresses/venues on a daily basis including transporting, clients, equipment & clinical stock required for treatment.

Main duties of the job

As a Primary Care Mental Health key-worker you will provide signposting, support and short-term interventions to promote mental health recovery. Establishing therapeutic relationships with service users and their family / carers. Provide clinical leadership within the team, ensuring high standards of care and supporting colleagues with complex cases.

Act as a role model for best practice and contribute to service development initiatives.

Offer guidance and supervision to team members and students, fostering a positive learning environment.

Support effective decision-making and problem-solving within the team to maintain safe, responsive care delivery.

Complete initial triage and assessment of needs including:

  • Signposting / referring clients to the most appropriate service to meet their needs.
  • Complete risk assessment and care planning documentation
  • Develop and deliver recovery focused care.
  • Provide information and psycho education surrounding mental health conditions and medication.
  • Uphold legal frameworks which underpin service delivery.
  • Assess risks, the management of risks and effectiveness of interventions.
  • Develop good working relationships with neighbouring teams to enable effective co-working and responsive transfers of care if necessary.
  • Monitoring of medication.
  • Offer leadership and support to the team in the absence of the Clinical lead day to day and in meetings.

Qualifiactions & Knowledge
  • Registered Mental Health Nurse on NMC live register
  • Demonstrable experience of mentoring pre-registration students
  • Full understanding of relevant policy, legislation, drivers and their application to clinical and service area, e.g. The Mental Health Act, Mental Capacity Act.
  • IT skills for learning systems required for role e.g. electronic patient record, clinical stock ordering systems, incident reporting systems.
  • oLeadership training/qualification/experience.
  • oEvidence of project work that has impacted on practice and demonstrates a higher level of communication/planning/change that impacts within own area of work/responsibility.
Experience
  • Previous community based experience or band 6 experience transferrable to community setting Experience of leading on complex care delivery
