This consultancy role pays up to £28.40 per hour and runs initially for three months, with the possibility of extension.   • Conduct long-term assessments and support hospital discharges for individuals on discharge-to-assess pathways.
  • Manage safeguarding and complex case scenarios, working efficiently within pressurised timescales.
  • Exhibit strong decision-making skills in positive risk-taking situations.
  • Collaborate effectively with various agencies to ensure comprehensive care.
  • Communicate complex and sensitive information through well-prepared reports and verbal updates.
  • Apply detailed knowledge of legislation such as the Care Act 2014, Mental Capacity Act 2005, and MHA 1983 to your practice.
