• Complete assessments and support plans according to the Care Act 2014, adopting a strengths-based approach
• Work collaboratively with the hospital team to facilitate smooth patient discharges
• Manage risks and support patients to enhance their independence
• Undertake strengths-based Care Act assessments for patients and their informal carers
• Participate in regular supervision and training sessions to continuously improve your skills
• Work in line with our Supervision Policy to ensure you receive guidance and support
