What You’ll Do:

As a key member of our interdisciplinary care team, you’ll support patients and families by providing emotional guidance, resource coordination, and advocacy in the hospice settings. You’ll collaborate closely with nurses, therapists, and physicians to ensure comprehensive care and a strong support system for every patient.

Your Day-to-Day Might Include:
  • Offering emotional and psychosocial support to patients and families
  • Helping patients and caregivers set healthy boundaries and goals
  • Identifying high‑risk situations and coordinating timely interventions
  • Referring families to community services and financial assistance programs
  • Supporting hospice patients with bereavement and respite care planning
  • Participating in team case conferences and care planning
  • Providing supervision to other social work staff (if licensed at LICSW level)
  • Documenting assessments, plans, and outcomes in a timely and accurate manner
