Overview

Social Worker - Assessments and Reviews Team in Luton, UK £30 hourly. Ongoing and full-time.

This role involves assessing the needs of adults and older people, ensuring they receive the appropriate care and support in line with the Care Act 2014. You will work with individuals, families, and multi-agency partners to promote independence, well-being, and safeguarding. You will be responsible for conducting initial and ongoing assessments, reviewing support plans, and working closely with health professionals, housing, and voluntary sector organisations to ensure service users receive the best possible care.

Responsibilities
  • Conduct comprehensive social care assessments under the Care Act 2014, identifying eligible needs and appropriate support.
  • Carry out statutory reviews to evaluate ongoing care needs and make necessary adjustments.
  • Work closely with service users, their families, and carers to develop person-centred care plans.
  • Identify safeguarding concerns and take appropriate action in line with adult safeguarding procedures.
  • Liaise with health professionals, housing teams, mental health services, and community support organisations to coordinate care.
  • Participate in multi-disciplinary meetings, ensuring a holistic approach to assessment and care planning.
  • Undertake safeguarding enquiries where there are concerns about neglect, abuse, or exploitation.
  • Support individuals in applying for funding, including personal budgets and direct payments.
Requirements
  • Social Work England Registration
  • Degree/Equivalent in Social Care
  • UK experience working as a qualified Social Worker, preferably in an Adults setting
