Job description

Overview

The Band 4 Healthcare Assistant (HCA) plays a vital role within the Richmond Urgent Community Response (UCR) team, supporting the delivery of rapid, high-quality care to patients in their own homes during periods of acute illness or functional decline. Working under the guidance of registered professionals, the Band 4 HCA provides delegated clinical and personal care tasks, supports assessments, and contributes to care planning and delivery. This role requires excellent communication skills, the ability to work flexibly and independently within a multidisciplinary team, and a commitment to promoting patient safety, dignity, and independence. The postholder will support timely interventions aimed at preventing hospital admission or facilitating early discharge, in line with the NHS UCR service model. Travel is an integral part of the post so applicants should possess the ability and willingness to travel across the borough for work purposes.

Responsibilities
  • Undertake delegated Rapid Response visits to patients in their homes.
  • Deliver a range of clinical and care interventions within the scope of Band 4 practice.
  • Support patients with acute and long-term conditions under the direction of registered professionals.
  • Perform basic assessments and communicate findings to the wider multidisciplinary team.
  • Assist in preventing hospital admissions by supporting stabilisation, recovery and independence.
  • Work as part of the multidisciplinary team, liaising with GPs, Virtual Ward, social care and voluntary sector colleagues.
