Your duties will include but is not limited to:
Devising treatment programmesto address the recovery needs and physical wellbeing of individuals withvarying mental health needs.
Undertaking mental health assessments and formulating careplans in conjunction with the MDT, patient, and their family/carers.
Identifying any specific precautions or contraindications to the proposedinterventions/treatments and taking the appropriate action.
undertaking and recording interventions/treatments correctly,and in line with legislation, policies, and procedures and/or establishedprotocols.
Ensuring that written records of all aspects of service user treatment andprogress are maintained throughout the treatment process.

Claimont Health is recruiting a fantastic full-time role as a Senior Complex Care Registered Nurse based in the community to support clients who have complex clinical needs and require a high level of care and support at home. The post holder will benefit from the support and motivation of both the clinical and operations teams and will provide support to long terms conditions and frailty clients in the comforts of their own homes.
The Post holder will undertake all aspects of clinical duties and will work within their professional scope of practice to deliver individualised care that supports patients to achieve a desired outcome. They will be responsible for the co-ordination and the delivery of a high-quality mental health care using evidence-based practice through the provision of clinical advice and support, acting as a role model, and providing education and training where appropriate. They will liaise closely with external professionals, family and carers, local services, and anyone else involved in the patients care package.
