Role Overview

Do you think you have what it takes to lead and manage a team of nurses striving to transform Community Care across the One Gloucestershire Integrated Care system (ICS)?

In this ever changing, modernising NHS environment we need nurses who are flexible, innovative and resilient enough to rise to the challenge of a very rewarding role. We seek nurses with compassion, who hold patient experience and high‑quality care at the heart of their nursing practice.

This is a community‑based role, visiting patients in their homes across the county and therefore a valid Driving Licence and access to a vehicle for business use is essential.

This role is offered on fixed‑term (or secondment) basis for 9 months. The hours are full‑time, 37.5 hours per week.

Key Responsibilities
  • Work within a dedicated referral centre, using well‑developed critical thinking and negotiating skills to aid decision‑making and triage process.
  • Manage a defined caseload of patients with delegated responsibilities to your Band 5 nursing team to support continuity of care for patients as part of the nursing model of care.
  • Act as an expert in your field in possession of extended competencies including vacuum‑assisted closure & syringe driver management.
  • Demonstrate excellent communication skills to illicit an accurate, holistic assessment of patient needs as part of a case management approach.
  • Support training and implementation of a Health Coaching approach to move towards greater engagement with patients regarding self‑management, particularly in those with long‑term conditions.
  • Work collaboratively with the newly merged Gloucestershire Health and Care NHS Foundation Trust services, and other professions within ICT, GP surgeries, Rapid Response and Complex Care at Home, ensuring the right care is delivered timely.
  • Maintain accurate electronic patient record IT systems (e.g. Systm One); prior experience is advantageous but full support and training will be given.
Requirements
  • Current NMC registration as a Registered Nurse.
  • Sound knowledge of evidence‑based care and best practice, and evidence of post‑registration development.
  • Extensive community nursing experience and a desire for career development.
  • A valid UK driving licence and own vehicle for business use. For qualifying staff the Trust has a Lease Car Scheme and also offers a Salary Sacrifice Car Scheme with partner Tusker.
  • Flexible, innovative and resilient mindset; excellent communication and case‑management skills.
  • A willingness to undergo a comprehensive induction plan, regular clinical supervision and leadership support.
