Job description
Reablement & Bridging Senior Care Officer

Full-time | 37 hours per week | rota between 7am-10pm | some home working depending on rota.

A distinguished opportunity has arisen for a dedicated and capable individual to join a highly respected Independent Living and Community Reablement service as a Reablement & Bridging Senior Care Officer. This role is ideal for an experienced care professional who is passionate about supporting adults to regain independence following illness or a hospital stay.

The Role

You shall serve as a senior member of the reablement team, entrusted with supporting more complex cases and guiding individuals to re‑learn essential daily living skills. Working in harmony with multidisciplinary colleagues, you will help shape effective, person‑centred plans that empower service users to achieve meaningful outcomes.

In addition, you will provide leadership to a cohort of Reablement Care Workers, ensuring that practice is safe, compliant, and aligned with the highest standards of regulated care. You will offer supervision, mentorship, and day‑to‑day guidance, fostering a positive and professional environment in which excellent service can flourish.

Key Responsibilities
  • Overseeing a designated team of Reablement Care Workers, offering formal supervision and maintaining high training and practice standards.
  • Monitoring individual support plans and ensuring that care remains suitable and responsive to each person's evolving needs.
  • Working collaboratively with locality leads and assessors to uphold robust procedures and quality benchmarks.
  • Delivering hands‑on, strengths‑based support for more complex cases, championing independence, dignity, and informed choice.
  • Maintaining meticulous records for both staff and service users.
  • Supporting colleagues with digital systems and day‑to‑day operational matters.
  • Upholding safeguarding responsibilities and promoting equality, safety, and environmental responsibility.
About You

To excel in this role, you will demonstrate:

  • Experience supporting older adults and/or individuals with disabilities.
  • A minimum QCF Level 2 in Health & Social Care and registration with Social Care Wales.
  • Experience supervising staff within a regulated care setting.
  • Strong organisational skills, with the ability to prioritise effectively under pressure.
  • Clear and compassionate communication, both written and verbal.
  • Proficiency with Office 365 and confidence using digital systems.
  • A person‑centred, outcomes‑focused approach to care.

A QCF Level 3 qualification, Welsh language ability, and a thorough understanding of CIW standards are desirable, though not required.

Additional Requirements
  • Flexibility to work across a rota, including early mornings, evenings, and weekends as required.
  • A full driving licence and access to a vehicle.
  • An Enhanced DBS check.
