  • Review and sign repeat prescriptions as required.
  • Follow and contribute to protocols for Quality and Outcomes Framework.

Overview

Hull Family Practice is a close knit, well organised and cohesive practice with a friendly working environment and a proven record of innovation and adapting to change. The practice is based across 5 sites located in the East and West of Hull. We are based in modern, purpose health centres that we share with a number of other NHS services and we believe in providing all our patients with the best possible experience and health outcomes.

The practice consists of 10 GP Partners, Nurse Practitioners, Paramedics, Pharmacists, Pharmacy Technicians, a Physiotherapist, a Social Prescriber, a Care Co-Ordinator, a Nursing team and a large Reception/Admin team.

Job responsibilities
  • Treatment of all patients
  • Perform telephone consultations, telephone triage and online consultations as needed
  • Support patients to adopt health promotion strategies that promote patients to live healthy and apply principles of self-care
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Perform home visits as agreed
  • Work with other health care professionals to diagnose, monitor, manage and treat long term conditions, including non-drug based treatment methods using a management plan and in line with national, local policies and practice needs
  • Conform to practice procedures for QOF, prescribing incentive schemes, PBC and other enhanced services and directives as stipulated by the National Bodies/CCG or Tudor Surgery
  • Complete referral letters as necessary for patients seen
  • Review and sign repeat prescriptions as and when required
  • Clinical Governance/Quality Assurance: Follow and contribute to protocols to achieve a high score on the Quality and Outcomes Framework
  • Enhance current policies and procedures within the practice as required
  • Ensure data is entered in an efficient and accurate manner on to the practice computer system
  • Promote confidentiality (in line with Caldicott principles) within the practice
  • Deal with non-NHS correspondence (such as life insurance, holiday cancellations and work related letters)
  • Deal with pathology results communications from Out of Hours Service
