A national healthcare service in Greater Manchester seeks a committed autonomous practitioner to provide high-quality medical services to our practice population. You will be responsible for assessing patients, developing care plans, and collaborating with a multidisciplinary team. Candidates must have an MRCGP qualification and experience in general practice is preferred. To work as anautonomous practitioner, responsible for the provision of medical services tothe practice population, delivering an excellent standard of clinical carewhilst complying with the PMS contract and other contractual and regulatoryobligations. The post-holder will adhere to the GMC standards for good medicalpractice, contributing to the effective management of the practice, leading byexample, maintaining a positive, collaborative working relationship with themultidisciplinary team
Makeprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation
Assessingthe health care needs of patients with undifferentiated and undiagnosedproblems
Screeningpatients for disease risk factors and early signs of illness
DevelopingCare Plans in consultation with patients and in line with current diseasemanagement protocols
Conduct reviewsof patients placed on the Practice Case Management List
Providing health education
Referringto other care providers as appropriate
Recording clearand contemporaneous consultation notes to agreed standards
Compilingand issuing computer-generated acute prescriptions (avoiding hand- writtenprescriptions whenever possible)
Prescribingin accordance with the local Medicines Management Board prescribing formulary(or generically) whenever this is clinicallyappropriate
Referrals shouldbe completed on a daily basis
Resultsare normally communicated either electronically or by letter and you will beexpected to action these on a daily basis for your own patients and on behalfof absent clinicians.

Youwill be expected to complete examinations and reports for the Benefits Agencyand those required under the PMS contract

Wehave a strong training ethos and take pride in training the primary careclinicians of the future.
