• Eligibility to enroll in CPPE Clinical Pharmacist Training pathway is required.

Responsibilities

• Contribute to health outcomes in long-term conditions.
• Provide specialist pharmaceutical advice.
• Review patients’ acute medication requests.
• Complete discharge and clinic letters.
• Conduct structured medication reviews as a PCN Pharmacist.
• Support patients with self-limiting conditions.
