A healthcare provider in Plymouth is seeking a motivated Registered Mental Health Nurse to join their team. You will support service users with complex mental health needs, engage in person-centred care, and work collaboratively in a multi-disciplinary team. We are looking for a motivatedRegistered Mental health Nurse to join our team at Syrena House who iscommitted to person centred care and has creative ideas and drive in order tosupport our service users who have enduring and complex mental health needs.
We are excited to be expanding our service and moving to a campus based building so lots of exciting changes for 2026.
Syrena house is currently a 9 beddedRecovery Unit and our aim is to support and prepare people for life in thecommunity and the transition required to achieve this.
The post holder will be aRegistered Mental Health Nurse with sound in-patient clinical experience,confident working in a multi-disciplinary team.
If you have a passion forpromoting recovery focused interventions and supporting people to reach theiroptimum level of functioning utilising resources within an in-patient settingwhilst linking with what the community and our stakeholders this might be thepost for you.
Additionally interests orexperience of impact of physical health and mental health issues and impact ofsubstance misuse use on mental health would be advantageous.

The Post holder will beresponsible for the assessment, planning, implementation and evaluation ofprogrammes of care using the Personalised Care Framework and will be requiredto act as a Keyworker for identified individual service users. The post holderwill be required to take charge of a shift on a regular basis.
The post holder will have theability to sustain respectful, consistent and reliable therapeuticrelationships with service users that enhances their ability to build socialnetworks and diminishes their social exclusion, maximise their strengths andinterests and increase their participation in meaningful activities.
The focus is to supportdischarge pathways for service users and to provide individualised care that issupported by co-produced personalised care. This needs to demonstrate evidenceof risk assessment and involvement of the relevant MDT members and carerinvolvement.
The post holder will be requiredto create and maintain a learning environment for the development of juniorstaff and be able to undertake the role of practice supervisor/assessor.
The post holder will also beresponsible for their own professional development.
The post holder will work at alltimes within the guidelines of the NMC Code.
Whilst the post is based atSyrena there may be an expectation to work within the wider Recovery Service atthe request of the Manager according to clinical and/or as part of personaldevelopment.

To deliver a high standard of individualised care, which willinclude the assessment, planning, implementation, review and evaluation of thePersonalised Care Framework. This should be in partnership with the serviceuser, carers and multi-agency professionals as appropriate. This requires acomplex level of communication in the receiving and transmitting of informationto service users, carers, colleagues, the wider service and the general publicon an individual or group basis. This will be over a wide range of complex andsensitive issues and will need to take into account barriers to understanding.
To act as Keyworker for an identified group of clients using thePersonalized Care Framework process and keep all associated paperwork up todate in a timely manner.
To ensure comprehensive risk assessment, history, planning andmanagement is recorded and communicated to the team and the wider service ifappropriate, for individual service users.
To contribute to and be an effective member of themulti-disciplinary team. This will include leading handovers and MDT meetings.Involvement in the assessment of new referrals and to attend and contribute toreviews, clinical meetings and case conferences. It is the role of the nurse toconvene necessary meetings, invite the relevant stakeholders, communicate andrecord, action and communicate outcomes.
The post holder will act as a role model to service users andstaff. There is an expectation that they will promote a recovery ethos toencourage service users in all aspects of activities of daily living. This willinclude providing verbal prompting and when required additional support toensure that all their basic physical health and personal care needs are met toa high standard. These interventions are also seen as opportunities forassessment/monitoring and/or learning opportunities.
The post holder will contribute toward the recovery process andthe care pathway by utilising their skills and knowledge in amulti-disciplinary way to provide holistic care. This will be achieved throughone-to-one work, group activities, networking and co-facilitation ofpsychotherapeutic groups. Contributing to the therapeutic programme within theunit and the creation of individual programmes for service users is a keyresponsibility.
To work in partnership with carers of clients using Triangle ofCare initiatives, offering support, advice and reassurance on often quitesensitive and emotional issues and at times of high levels of distress.
Maintain and regularly update knowledge of The Mental Health Act1983, and be aware of own responsibilities and clients rights under the Act. Towork at all times within the Code of Practice and Livewell Southwest policies
To be responsible and accountable for the safe storage and administrationof medication as per national and Livewell Southwest policies and procedures.To educate clients and carers regarding the potential benefits and side effectsof medication and feedback information to the medical staff/communitypharmacist.
The post holder is required to comply with all infection controlpolicies which includes adherence to bare below the elbows.
Leadership
To take responsibility for the Nurse in Charge role of the unitin the absence of more senior staff, including the planning and organisation ofthe daily clinical activity, covering adjustment of staff rotas during the outof hour period and recording as necessary, flexible changes to routines inresponse to individual service users need and changing staff levels and tomaintain the safety of staff and clients.
To contribute to staff development and demonstrate workactivities to new or inexperienced staff e.g. contribute to induction of newstaff when joining a team/temporary staff who are working shifts.
Provide advice for staff and direct to appropriate resourceswhich facilitate evidence-based nursing practice.
To be aware of and appropriately implement the policies andprocedures relevant to Livewell southwest and keep up to date with theircontents.
To be responsible in attitude towards economy of resources andcare of equipment and property including the day-to-day financial management ofpetty cash; safe storage and recording of patient monies; stock control of allsupplies i.e. stationary, medicines, food office supplies etc and the healthand safety of the environment.
To be responsible for the safe use of unit vehicles and theaccurate recording of mileage. To be responsible for carrying out riskassessments and planning to reduce risk when transporting patients.
To undertake appraisal and line management supervision of juniorstaff as allocated or as part of ongoing personal development.
Professional Development and Education
The post holder will undertake a comprehensive induction,mandatory and other essential training as directed by the Livewell Southwestand Unit/Deputy manager. This will include Physical Intervention Training whichis essential for this post.
Competencies will be identified through personal developmentneeds and appraisal.
The post holder will be responsible for their own practice,managerial development and to keep up to date with evidence-based developmentsin mental health nursing. This will include post registration education andpreparation of portfolios for revalidation.
The post holders profession will be maintained and support foropportunities and the ways that their skills and knowledge can be used to meetrequirements for revalidation will be central to the line management andappraisal process.
The post holder will be required to ensure they receive linemanagement support and participate in clinical supervision and reflectivepractice.
Clinical Governance
To understand responsibilities under clinical governance tocomply with Essential Standards of Quality and Safety as regulated by the CareQuality Commission.
To contribute to any audit process within the Livewell Southwestas appropriate.
Corporate Responsibilities
The post holder will behave in a manner that does not bring theservice and Livewell Southwest into disrepute.

Infection Control
Livewell Southwest is determined to eradicatehealthcare-acquired infection and puts a great deal of emphasis on theresponsibility of all staff to ensure their own personal and others compliancewith Infection Control (including Hand Washing) Policies.
All staff must comply with infection control policies andguidance, attend relevant updates and report issues of concern to theirimmediate line manager (if no action or explanation received, then it is theindividuals responsibility to escalate their concerns to the Director ofOperations or Chief Executives Office).
Safeguarding Children
All employees have a duty for safeguarding and promoting thewelfare of children and are required to act in such a way that at all timessafeguards the health and well-being of children. Familiarisation with andadherence to national and local Safeguarding policies is an essentialrequirement of all employees as is participation in related mandatory/statutorytraining.
Research
For clinical posts at bands 5 and 6, there is an expectationthat engagement in research will be part of this role. Therefore, an awarenessof the value and relevance of research is expected, and the post holder shouldeither support existing research within their speciality area as appropriate oridentify opportunities to raise awareness of research possibilities.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
