Rate: £33 – £35 per hour
An exciting job role has recently become available in Lancashire. We are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be?
You will conduct assessments to understand patients’ psychosocial needs and preferences before discharge. You will coordinate and plan comprehensive discharge strategies with healthcare teams. You will arrange post-discharge services and provide education and emotional support to patients and their families to ensure a smooth transition from hospital to home or another care setting.

Qualifications and Experiences:
You must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
