Adult's Social Worker - Hospital Team - BCP - £36 ph

• Location: Bournemouth (Hybrid – Bournemouth Hospital)
• Rate: Up to £36 per hour
• Contract: Full-time

We are currently seeking an experienced professional to join our team on a hybrid basis, working at Bournemouth Hospital 2–3 days per week. This role requires strong experience within a hospital setting and a solid understanding of Discharge to Assess (D2A) pathways.

Key Responsibilities

• Work collaboratively within a hospital environment to support safe and timely discharges
• Apply Discharge to Assess principles to ensure effective patient flow
• Liaise with multidisciplinary teams, patients, families, and external agencies
• Ensure assessments and decisions are person-centred and outcomes focused
• Maintain accurate and timely records in line with policies and procedures

Requirements

• Proven experience working within a hospital setting
• Clear understanding and practical experience of Discharge to Assess (D2A)
• Ability to work on-site at Bournemouth Hospital at least 2–3 times per week
• Strong communication and organisational skills; Ability to work effectively in a fast-paced environment
