The Home to Decide Team helps residents return home from hospital, fostering independence and reducing the need for residential care. The Resilience Service forms the umbrella for this team, helping to reduce hospital discharge delays and ensure continuity of care.

Responsibilities

• Provide support and supervision to social work staff and associated personnel in the Home to Decide Team, maintaining high standards in all aspects of social care.
• Screen and allocate referrals effectively, ensuring clients receive appropriate and timely support.
• Handle complex casework with confidence and professionalism, guiding staff through intricate cases with a focus on safeguarding and best outcomes for clients.
• Participate in the Duty Rota, acting as the senior practitioner based in the office, with minimal home working to ensure consistency and availability for urgent needs.
• Foster independence among residents, supporting them to achieve personal objectives and return to their previous level of autonomy.
