Practice Nurse – Pucklechurch - 38,000 - 57,500 per annum

We are looking for a dedicated Practice Nurse to join our friendly, supportive team in Pucklechurch. This is an exciting opportunity to play a key role in delivering excellent nursing care to patients of all ages, while being part of a practice that values teamwork, innovation, and patient-centred care.

About the role

As a Practice Nurse, you’ll be involved in all aspects of primary care nursing. Your day might include managing chronic disease clinics, supporting patients to make positive lifestyle changes, delivering vaccinations, advising on travel health, or providing wound care and screening services. You’ll also play an important part in health promotion, working with patients to prevent illness and helping them feel confident about their health choices.

This is a role where no two days are the same. You’ll be part of a multi-disciplinary team who work closely together to ensure our patients get the very best care. You’ll also have the opportunity to contribute to the ongoing development of our services, including well woman and well man clinics, and the review of clinical policies and procedures.

Key responsibilities
  • Running and supporting chronic disease management clinics, referring patients when necessary.
  • Delivering immunisation and vaccination programmes for children and adults.
  • Providing wound care, including ulcer and Doppler assessments.
  • Offering travel health advice and vaccinations.
  • Requesting and processing pathology tests and results.
  • Supporting patients with mental health needs and safeguarding concerns.
  • Encouraging self-care and healthy lifestyle choices through health promotion.
  • Maintaining accurate clinical records and ensuring effective use of clinical systems.
  • Helping to shape and improve services such as well woman and well man clinics.
What we’re looking for

We’re seeking a nurse who is compassionate, proactive, and organised, with excellent communication skills and a patient-first approach. You’ll be confident working independently but also enjoy being part of a supportive team. Experience in chronic disease management, immunisations, and general practice nursing would be an advantage, but we’re equally keen to hear from nurses who are looking to develop their skills further.
