Job description

Colchester Medical Practice has a fantastic opportunity for an experienced practice nurse to join our team on a full time or part time basis, we are open to discussion on hours and shifts. The role will be predominately based at Shrub End Surgery, although may require cross cover across our sites.

Responsibilities

• Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
• Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
• General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
• Managing and treating long-term conditions, in line with national and local policies and the practice needs.
• Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
• Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
• Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
• Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
• Meeting the needs of patients presenting for opportunistic wound care.
• Hypertension review and management.
• Running asthma, COPD and diabetes clinics and reviews.
• New patient and health checks.

Colchester Medical Practice is a well established PCN, delivering excellent care to over 42,000 patients across the city of Colchester. We have four sites (Shrub End Surgery, Castle Gardens Medical Practice, Wimpole Road Surgery and Parsons Heath Medical Practice) and a new health and wellbeing hub, based at our Parsons Heath site. The practice is well established with a rating of GOOD by the Care Quality Commission. As a practice we employ over 120 colleagues to provide excellent healthcare to patients across Colchester.
