Overview

Practice Nurse - Cherry Hinton & Brookfields Medical Centre

Are you a passionate Practice Nurse? An exciting opportunity is available for an experienced Practice Nurse in Cherry Hinton & Brookfields Medical Centre.

About Cambs GP Network – a Federation of Practices across Cambridge and surrounding villages, with a social enterprise ethos. We hold the contract to deliver services at Cherry Hinton & Brookfields Medical Centre. The centre serves nearly 10,000 patients with a collaborative clinical team and in-house support to reduce GP workload. The role supports face-to-face, high-quality consulting and is backed by a larger network for stability.

Work pattern: 36 hours per week (part-time discussion possible). Includes 1 monthly clinical closure for training and team building. SystmOne experience with Ardens templates and NHS Pension benefits apply.

Responsibilities
  • Deliver safe, effective nursing care to the whole practice population and be accountable for nursing service delivery.
  • Engage in practice and patient engagement, supporting policy and procedures across the team.
  • Provide evidence-based care for patients with long-term conditions, focusing on management, prevention, and outcome improvement.
  • Assist with infection prevention and control compliance; assure clinical safety and optimize patient outcomes.
  • Coordinate care for patients with complex health needs; liaise with other healthcare professionals and community services.
  • Administer vaccinations and immunisations (including travel where competent); follow national guidelines and practice protocols.
  • Support women’s health needs, including contraception and cervical screening.
  • Perform complex wound management, venepuncture, and request pathology services as required.
  • Respond to acute clinical requests and emergencies; act as chaperone as needed; support safeguarding in line with local and national policies.
  • Collaborate with multidisciplinary teams to develop and implement holistic care plans.
  • Be flexible in interpreting the above duties as the Practice Nurse role evolves.
Qualifications and Experience
  • Essential
    • Registered Nurse with valid NMC registration.
    • Experience of working in a primary care environment; experience as a practice nurse or community nurse.
    • Excellent communication skills; strong IT skills; SystemOne experience; effective time management.
    • Ability to work as part of a team and autonomously; problem solving and analytical skills; ability to follow clinical policy and procedures.
  • Desirable
    • Experience in chronic disease management; travel medicine; diabetes; hypertension; CHD; respiratory (asthma & COPD).
    • Experience with audits.
