A healthcare provider in Bacup is seeking a dedicated Registered Nurse to join their practice. The ideal candidate should have experience in chronic disease management and nursing within a general practice setting. Responsibilities include supporting patients, managing long-term conditions, and delivering health promotion activities. The post holder is responsible for the delivery of practice nursing services to the practice population. Supported by the Nurse Manager, they will deliver care within the boundaries of their role, working within practice protocols, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. The duties will include all tasks normally undertaken by an experienced RN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of practice policies and procedures and will also need to have an understanding of the public health profile and population needs in order to be proactive in ensuring services are met.

Ability to assess and manage the range ofconditions encountered in general practice using a variety of assessment toolsand consultation models appropriate to the patient and situation using physicaland clinical examination skills to inform theassessment and decision-making forthe on-going managementof the patient.
Advanced communication skills that includeskills of influencing and negotiation to enable information to be delivered inunderstandable formats for patients and behaviour change supported wherenecessary.
Effective multidisciplinary and multi-agencyteam working, alongside the ability to work independently and acceptprofessional accountability and responsibility for the delivery of wholeepisodes of care, and supporting and developing others in the general practicenursing team to collaborate effectively, ensuring nursing care is guided byprecedent and clearly defined policies, procedures and protocols.
Deliveryand co-ordination ofevidence-informed, person-centred and negotiated care across the agespectrum.
Use of technology to support independenceand patient self-care at home to improve self-care and reduce exacerbations
