Are you passionate about transforming healthcare and making a real difference in people’s lives? We’re seeking a dynamic Population Health Management (PHM) Senior Nurse/AHP to lead the charge in Oldham East’s innovative health improvement initiative.
About the Role: You’ll be a vital part of a forward-thinking team dedicated to proactive, preventative care for adults aged 50-64 who are at risk of frailty. Working across the PCN, you’ll collaborate closely with a dedicated care coordinator to identify, engage, and support individuals early—helping them maintain their health, manage chronic conditions, and prevent deterioration.
