Overview

Are you looking for a new challenge whilst working within the Community?

We are looking for someone to act as a source of expertise on the assessment and management of patients presenting with a wide range of pathologies to physiotherapists, student physiotherapists, skilled non-registered staff, other healthcare professionals, GPs, Consultants, patients and their carers.

Role and Location

Able to provide a high standard of physiotherapy service within Torbay. This will involve community rehabilitation, crisis response and Intermediate care cover. You will be required to work between these areas dependant on needs of the service, this will take part in the Baywide Intermediate Care weekend rota and late duty rota within the zone. This may also include from time to time, covering other localities within the Trust.

Responsibilities
  • Deliver high quality care to the housebound patients of Torbay and work as part of multi-disciplinary team and in partnership with primary care and secondary care.
  • Undertake all aspects of clinical duties as an autonomous practitioner, including professional and legal accountability and managing clinical risk for all aspects of own work.
  • Some aspects of work will involve delivery of the care as part of a multidisciplinary team.
  • In most cases to be the sole health professional with responsibility for that patient9s episode of care.
  • Undertake advanced physiotherapeutic assessment of patients with diverse or complex physical, psychological, cognitive and behavioural conditions in order to provide an accurate diagnosis and prognosis.
  • Formulate and deliver individualised physiotherapy treatment programmes, including manual therapy techniques, therapeutic handling, patient education, electrotherapy, provision of exercise (individual/group), and other alternative options.
  • This includes the appropriate selection of referrals for inclusion in a range of treatment programmes, one to one sessions using advanced treatment options, advanced communication, questioning and reasoning skills.
