Job description
Overview

We are looking for an enthusiastic and motivated Physical Health Support Worker to join the Later Life Team. The team supports adults over 65 with secondary mental health needs and Memory Assessments. We cover the South Manchester area and support people in their own homes, sheltered/Extra Care Housing or in care homes. The post does not require professional registration, but experience of mental health is essential. The post holder's professional development will be supported by the service through regular supervision and training opportunities.

Responsibilities
  • Complete physiological assessments of service users, including taking and recording physical observations such as blood pressure, pulse and weight. Venepuncture training will be provided for the right candidate.
  • Support the Memory Treatment pathway by undertaking relevant observations.
  • Assess service users holistically and complete Physical Health Assessments in their homes.
  • Be an active and contributing member of the Community Mental Health Team and provide feedback to the MDT as needed.
  • Provide information on health promotion which may include smoking cessation, alcohol use and dietary needs appropriate to the client group.
