K&W Healthcare is seeking an experienced and motivated Paramedic to join our growingPrimary Care Network (PCN) team under the NHS England Additional Roles ReimbursementScheme (ARRS). Main duties of the job

As a PCN Paramedic, you will deliver first-contact care to patients presenting with a wide range of minor illnesses, injuries, and complex or undifferentiated conditions. You will assess, diagnose, manage, and treat patients independently, referring appropriately and promoting self-care, health education, and wellbeing.

You will assess and triage patients, providing definitive treatment or onward referral within the PCN. The role includes advising patients on general health, self-management, and signposting to community and voluntary services. You will undertake specialist health checks and diagnostic procedures such as ECGs and interpretation, pathological specimen collection, and other investigations as required.

You will support anticipatory care planning, lead specific community services (e.g. BP and diabetes monitoring), provide alternatives to urgent or same-day home visits, and contribute to clinical audits. Effective communication and collaboration with GPs, nurses, allied health professionals, and external organisations is essential.

You will maintain accurate patient records in line with GDPR, prescribe and review medications under PGDs and NICE guidance, support medication adherence, and manage both acute and chronic conditions using pharmacological and non-pharmacological approaches. You will prioritise urgent and emergency situations and promote healthy lifestyles and self-care strategies.

Job responsibilities
  • Assess and triage patients, providing definitive treatment or appropriate referral to other PCN team members.
  • Advise patients on general health care, self-management, and signposting to community or voluntary services.
  • Conduct specialist health checks, reviews, and diagnostic procedures including:
    • ECGs and interpretation
    • Collection of pathological specimens (blood, swabs, etc.)
    • Other investigatory procedures as required by patients or GPs
  • Support anticipatory care plans and lead specific community services (e.g., monitoring BP and diabetes in sheltered housing).
  • Provide alternative models to urgent or same-day home visits and participate in clinical audits.
  • Communicate effectively across PCN teams and external organisations, ensuring patient-centred service delivery.
  • Collaborate with GPs, nurses, and other allied health professionals, managing referrals appropriately.
  • Maintain accurate, contemporaneous patient records, in line with GDPR and confidentiality guidelines.
  • Prescribe, issue, and review medications following PGDs, NICE guidance, local protocols, and evidence-based practice.
  • Support patient adherence to prescribed treatments and provide advice on medication regimens, side effects, and interactions.
  • Assess, diagnose, plan, implement, and evaluate care for patients with undifferentiated or complex presentations.
  • Perform clinical examinations from physiological and psychological perspectives to plan care.
  • Prioritise and manage complex, urgent, or emergency situations, including initiation of emergency care.
  • Manage both acute and chronic conditions, integrating drug and non-drug interventions.
  • Promote health and well-being, supporting patients to adopt healthy lifestyles and self-care strategies.
