A local healthcare provider in Swansea is seeking a Patient Coordinator to join their patient-focused team for 18 hours a week. The role involves dealing with patients in person and over the phone, supporting the clinical team, and undertaking administrative duties. Candidates should have strong communication and IT skills, a background in General Practice, and the ability to work well under pressure. We are seeking toappoint a Patient Co-ordinators to work 18 hours a week between 8:00AM - 6:30PM with our friendly patient focused teamat Ystradgynlais Group Practice.
We are looking foran enthusiastic, reliable, highly motivated and friendly individuals to joinour team. The successful candidates would ideally have experience working inGeneral Practice.
Thesuccessful candidate will be a key member of the patientco-ordinator teams. You will play a vital role in dealing with the patients,both face to face and over the telephone. You will provide support the GPs andclinical team by signposting patients to the appropriate healthcareprofessional or service. The patient coordinator will also be expected toundertake reception duties and support the administrative team. You will berequired to work across our 3 sites.
We operate across 3 sites Pengorof Surgery, Ystalyfera Surgeryand Abercrave Surgery. Our team of 9 Partners, 3 Registrars are supported by 1Advanced Nurse Triage Practitioner, 3 Advanced Care Planning Nurses, 3 PracticeNurse, 3 Health Care Assistants, 2 Practice Pharmacists, 3 Phlebotomists, 1 Cluster Pharmacistand 2 Cluster Pharmacy Technicians and 4 Pharmacy dispensers. Practice Manager,2 Deputy Managers and a large team of administrative/clerical staff.
Our practice aims to improve the health of those living in andaround the Ystradgynlais area and we work closely with many local communityservices to achieve this, including the community hospital where our GPsprovide cover to the Minor Injuries Unit and work alongside the wardConsultants.
To support the practice clinical team by signposting patients to theappropriate healthcare professional or service, working as part of the practicesmultidisciplinary team. The care navigator will also be expected to undertakereception duties as part of the role, supporting the administrative team indelivering a polite and professional service to the entitled patientpopulation.
