Job Role

Title Community Health & Social Care Hub – Patient care Clinician

Band 5 (£ 31,048 - £37,796)

Hours 22.5 hours -37.5hrs, negotiable

An exciting opportunity has arisen for the right candidate to join our clinical team within our community health & Social Care Hub.

Responsibilities
  • Provide high quality clinical advice and support to patients, carers and professionals in contact with the CH&SCH.
  • Support administrative colleagues to ensure seamless, safe advice, support and care in line with the Maximising Independence agenda.
  • Ensure assessments and care plans are recorded appropriately in line with Locala and NMC record keeping / HCPC Professional standards.
  • Ensure clinical advice, support and care meets acceptable standards of patient safety and quality outcomes.
  • Respond to telephone enquiries as appropriate, assess health care needs of patients, resolve some at source and ensure smooth transfer to others in accordance with clinical need.
  • Work with a high degree of autonomy and professional accountability.
  • Work closely with admin colleagues in CH&SCH to maximise patient/carer independence and prevent unnecessary referrals to locality/expert teams.
Job Summary

To provide high quality evidence based clinical advice and support to patients and carers contacting the CH&SCH. To support the role of the Team Leader - Clinical. To support administrative colleagues in the CH&SCH. Be required to work in community to maintain clinical skills and competency and in the event to meet service need.

The post holder must have effective communication skills and must be prepared to work flexibly and collaboratively to meet the needs of the service over a 7 day working pattern to meet the needs of the service.
