Overview

We are a Gold standard practice with a diverse clinical team, training practice and committed to healthcare for all. We teach medical students and GP registrars, and engage with the local community including homeless, asylum seekers, and individuals affected by drugs and alcohol addiction.

Location: Waldron Health Centre, Amersham Vale, New Cross, London SE14 6LD. The post is a permanent, full-time role for a Paramedic Practitioner.

Main duties
  • Provide clinical care within a team of general practitioners, advanced healthcare professionals, practice nurses and Healthcare Assistants.
  • Patient contact mainly via triage, on-call duties and home visits.
  • Deliver duties including home visits, post-discharge reviews and minor illness management.
  • Provide care for presenting patients from initial history taking through clinical assessment, diagnosis, treatment and evaluation of care.
  • Demonstrate safe, clinical decision-making and expert care, with appropriate assessment and diagnosis skills for patients in primary care.
  • Work as part of a multidisciplinary team to deliver safe and high quality care.

Role responsibilities include prioritising and triaging patient needs, referring for investigations, managing caseloads and discharging patients as appropriate, and working within the scope of a qualified Paramedic Practitioner.

Job responsibilities (duplicate sections consolidated)
  • Operate autonomously within professional scope; provide care face to face, by telephone, and via home visits.
  • Coordinate care with a clinical lead, GPs, practice nurses and HCAs; conduct clinics, triage, on-call duties and home visits.
  • Deliver care aligned with NSF, NICE guidelines and evidence-based practice; perform specialist assessments and referrals as needed.
  • Prescribe and review medications appropriate to patient needs within scope and protocols.
  • Diagnose and manage acute and chronic conditions; order and interpret diagnostic investigations.
  • Involve patients and carers in planning and delivery of care; refer to multidisciplinary teams and secondary care when necessary.
  • Maintain professional standards, engage in CPD, participate in audits and practice change to meet patient needs.

Qualifications (Essential)

  • Qualified paramedic or specialist paramedic (FDSc or BSc) registered with HPCP or Level 3 in first contact care
  • Chronic disease management and mental health experience
  • Completion of Stage 1 and Stage 2 of Health Education England (HEE) Practice Mapping

Qualifications (Desirable)

  • Accredited training in chronic disease management
  • Non-Medical Prescribing qualification (independent/supplementary)
  • Mentoring/teaching qualification (MSC level)

  • Ability to analyse and interpret complex information, pre-empt and evaluate issues and recommend appropriate actions
  • Independent thinker with good judgement and problem-solving skills (Desirable)
