A healthcare provider in Chard is seeking a Paramedic (FCP) to enhance patient outcomes and tackle health inequalities. As an Paramedic (FCP) you willbe acting within your professional boundaries, providing care for patients frominitial history taking, clinical assessment, diagnosis, treatment andevaluation of their care with the support of the other Practice staff members asrequired. You will demonstrate safe,clinical decision-making and expert care for patients within the Primary CareNetwork (PCN), whilst working autonomously and taking full accountability forpatient encounters. You will work withinyour professional boundaries as part of the Multi-Disciplinary team (MDT) withinChard, supporting the delivery of the Primary Care Directed Enhanced Service tomeet the needs of patients. You could beworking alongside our home visiting and care home team or conducting urgentcare clinics.
CLICK (Chard, Ilminster andLangport) Primary Care Network (PCN) is located in South Somerset with 5 GPpractices and a diverse population spread across town and rural locations. CLICK has a population size of around 50,000patients.

As a PCN we are forward thinking, innovate and driven todeliver the best patient care for our population. This includes healthpopulation management, and this role ties in with supporting that and trackingthe improvements we can make to patients lives.
Assess and triage patients,including same day triage, and as appropriate provide definitive treatment ormake necessary referrals to other team members
Advise patients on generalhealthcare and promote self-management where appropriate, including signpostingpatients to other community or voluntary services
Be able to:
Provide an alternative modelto urgent and same day home visits for Chard and undertake clinical audits
Communicate at all levelsensuring effective, patient-centred service
Communicate proactively andeffectively with all colleagues across the team, attending and contributing tomeetings as required
Collaborate with doctors,nurses and other AHPs, accepting referrals and referring to them for specialistcare
Maintain accurate andcontemporaneous health records appropriate to the consultation, ensuringaccurate completion of all necessary documentation associated with patienthealth care and registration with the practice
Prescribe, issue and reviewmedications as appropriate following policy, patient group directives, NICE(national) and local clinical guidelines and local care pathways. This will bedone in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice
Work with patients in order tosupport compliance with and adherence to prescribed treatments
Provide information and adviceon prescribed or over-the-counter medication on medication regimens,side-effects and interactions
Assess, diagnose, plan,implement and evaluate treatment, interventions and care for patients presentingwith an undifferentiated diagnosis, and patients with complex needs
Clinically examine and assesspatient needs from a physiological and psychological perspective, and planclinical care accordingly
Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care
Diagnose and manage both acuteand chronic conditions, integrating both drug- and non-drug-based treatmentmethods into a management plan
Support patients to adopthealth promotion strategies that promote healthy lifestyles, and applyprinciples of self-care
Identify risk issues thatimpact on peoples health or social care needs.
Take appropriate action to thesignificance of the risk and consistent with protection procedures, applyingprotection procedures, following lone worker procedure.
Demonstrate effective teamworking inclusive of all relevant professionals.
Report all accidents, incidents, and all ill health, failings in equipment and or environment toline managers.
Contribute towards audit anddata collection as required.
Once assessed as competentwill be accountable for their own practice within their area of responsibilitywhen identified and agreed with the line manager.
Work with your line manager toundertake continual personal and professional development, taking an activepart in reviewing and developing the roles and responsibilities.
Adhere to organisationalpolicies and procedures, including confidentiality, safeguarding, lone working,information governance, and health and safety.
Work with the Clinical Lead toaccess regular clinical supervision, to enable you to deal effectively withthe difficult issues that people present.
Work as part of the team toseek feedback, continually improve the service and contribute to businessplanning.
Undertake any tasks consistentwith the level of the post and the scope of the role, ensuring that work isdelivered in a timely and effective manner.
