A healthcare organization in Armthorpe is seeking a Senior Community Mental Health Nurse to provide high-standard clinical practice in a supportive team. The role involves promoting nursing care in community settings and working closely with various agencies to support patients with complex issues. Working as a senior member of an established Community Mental Health Team in the East Locality of Doncaster providing a high standard of safe, robust and evidence based clinical practice.
Working with complex organic and functional diagnosis within the community setting, liaising and integrating with both statutory and voluntary organisations in support of the specific client group.

To promote and maintain the highest attainable standard of nursing care in a community setting.
To support the band 6 nursing team with implementation of evidence based interventions from a plan of care.
Perform re-assessments of clients to include risk and crisis plans.
To maintain close working relationships with referring agencies.
To deputise for the Team Leader when required.
To carry a caseload of patients with both functional and organic issues as identified through the team systems process.
