A healthcare provider for mental health services in Rochdale is seeking a Registered Mental Health Nurse to join the Home Intensive Treatment Service for Older Age Adults. The role involves conducting holistic assessments, providing support in crisis situations, and collaborating with multidisciplinary teams. Candidates must have strong skills in mental health care and crisis intervention. This position offers the opportunity to make a significant impact on the lives of older adults experiencing mental health issues.
Rochdale Home Intensive Treatment Service for Older Age Adults provides an alternative to hospital admission for residents of Heywood, Middleton and Rochdale who are predominantly over the age of 65 and are experiencing mental health crisis. The team also supports early discharge from the inpatient wards. The team is comprised of Registered Mental Health Nurses, Team Manager and Team Administrator.
To undertake holistic mental health and risk assessments using Trust approved documentation and procedures. To support clients who are in mental health crisis who will have a variety of mental health conditions, both organic and functional. To provide evidence based therapeutic interventions promoting enablement, choice and recovery. To comply with all legislative requirements in relation to service users care. Actively participate in multi-disciplinary meetings and contribute to clinical decision making. Engage, supervise and monitor junior staff and student nurses.
