Overview

Due to promotion, an exciting opportunity has arisen for an Occupational Therapist (OT) to join the In Patient Therapies Team at the Stonebow Unit in Hereford. You will work as part of a dedicated multi-disciplinary team to provide OT assessment and treatment for a variety of patient groups including older adults with dementia, functional older adults, and working age adults, based on skill set and team need. The OT service utilises the Model of Human Occupation as the informing evidence base, using standardised assessment tools and evidence-based intervention plans. The team includes Senior OTs, Therapy Assistants, Physiotherapy and Psychology. There is a strong emphasis on training, supervision and reflective practice with an expectation for clinical excellence. You will have a passion for working in mental health services, excellent communication and interaction skills and motivation to develop your practice.

This is a full-time post, but we would consider applicants looking for part time.

Responsibilities
  • Complete formal assessments to reduce the risk of falls.
  • Empower patients to maintain skills for meaningful occupation.
  • Consider the impact of task demands, life history and environment on patient wellbeing.
  • Support the MDT in making recommendations for discharge, including home environment assessments (a car and valid driving licence will be required).
  • Offer 1:1 and group interventions focusing on occupation for wellbeing, sensitive to cognitive ability.
  • Liaise with wider Mental Health services within the community, external agencies, families and carers to gather information and support discharge.
  • Complete accurate records and detailed occupational therapy reports, and support training and supervision of assistant staff and students.
