Job description
Overview

The newly formed Out of Hours Urgent Community Response (OOH UCR) team combines the patients traditionally seen by both District Nursing and our Intensive Home Support Service. This is an exciting time to join the service as we come together to enhance the level and breadth of care and treatment we are able to deliver to our patients in their own home, preventing unnecessary hospital admissions and meeting the national guidance around Urgent Community Responses. A permanent Band 6 position is available for an enthusiastic and dedicated registered nurse. For the successful candidate this is an excellent opportunity to work in a supportive environment as a Team Sister, developing high level skills in patient assessment and care and contributing to leadership within the team. This post is focused on the out of hours night nursing service, which currently works between 21:00 and 08:00 every night. Occasional day time work will be required to meet your learning and development needs. This is a 7-day service, including weekends and Bank Holidays. Operating across our two bases at Burnley General Hospital and St Ives House, Blackburn, the post holder must demonstrate professional standards of practice and personal qualities and behaviours as expected by the NMC Code and be passionate about high quality patient care. Responsibilities
  • Assists the Team Leader in the co-ordination and delivery of care in line with nation and local guidance.
  • Assessment of new and complex patients
  • Delivery of comprehensive Senior Nurse Reviews of patients in their own homes and other community care settings
  • Working as part of a wider multidisciplinary team
  • Supporting students and new staff in learning and development in the service
  • Development of the team through leadership and role modelling
  • Management of staffing rotas and allocation of clinical work
  • Establishing and maintaining effective communication with all clinical staff, patients and families
  • You will have the ability to work contracted hours rotationally across day and night shifts, as reasonably required by the needs of the service.

The post holder must demonstrate professional leadership and be passionate about patient care. They will need to be able to problem solve and adapt to challenging situations, manage resources efficiently and ensure the delivery of safe, personal and effective patient care. We are looking for a nurse with a commitment to quality care and the ability to positively challenge to ensure patient and staff safety. Car driver with full UK driving licence and access to a vehicle is essential.

Qualifications
  • Registered Nurse i.e. RN Adult branch
  • Community Specialist Practitioner Qualification or willingness to work towards this
Desirable criteria
  • Non-medical prescriber V100/V150/V300
  • Leadership training
  • Level 6 study to support service delivery and development
  • Mentorship or Coaching qualification, substantial district nursing experience
  • Experience in holistic assessment of complex patients and care coordination
  • Experience of multidisciplinary team working
  • Experience of caseload management and work allocation
  • Experience of service development/supporting change in practice, ability to use information technology effectively
  • Ability to develop effective working relations with other agencies and professionals
  • Ability to manage own and others workload and delegate effectively, car driver with full UK driving licence and access to a vehicle for work and insured for business purposes
  • You must have appropriate UK professional registration.
