About the role

A Part‑time (30 h, Mon‑Fri) Anticoagulation team member will manage all aspects of patient anticoagulant treatment within a well defined dosage competency framework. The role involves active education, dose advice and liaison with clinicians, nurses and patients at all points along the anticoagulation pathway.

Responsibilities
  • Manage the anticoagulant pathway from referral through to treatment implementation and ongoing warfarin dose management.
  • Provide initial and ongoing patient specific education for warfarin and DOACs following national guidance.
  • Liaise with other health‑care providers to ensure patient‑centred safe anticoagulation management.
  • Act as patient advocate and keep the GP appraised of all warfarin dose advice.
  • Develop effective working relationships with all service sites including Surgery and Medicine, Central Booking Service, patients and carers.
Qualifications & Experience
  • Self‑motivated, capable of working independently and within a team.
  • Experience in managing patients on an anticoagulant with knowledge of warfarin pharmacokinetics is an advantage.
