Neighbourhood Care Coordinator - Grantham

The Neighbourhood Coordinator will work with the Head of Integrated Neighbourhood Working and Senior NeighbourhoodCare Coordinator, member practices and key local operational stakeholders to support coordination of care for patients through a multidisciplinary team meeting approach, stakeholder signposting and supporting the across the whole South West footprint.

Key outcomes of neighbourhood working have been identified as:

Enabling people to remain in, or close to, their own home, whenever possible

Improving pathways between services, reducing inefficiencies, fragmentation, and duplication of care

Improved communication and working relationships between individuals and providers

Improving access to, and information about services

Continuity of care

Increasing out of hospital care provision

Main duties of the job

We are looking for individuals with proven experience in health and care environments or equivalent experience and evidence of continued personal development.

You will also have excellent interpersonal and communication skills, be highly motivated and positive in your approach to care delivery to ensure we maintain a high quality service delivery to our population.

Our Neighbourhood Team Coordinator posts are an exciting new addition to our already existing model of local population based health and care. The roles will support our Primary Care Networks in the Sleaford and Grantham localities of South West Lincolnshire, being aligned to our established, integrated Neighbourhood Team.

The Neighbourhood Coordinator will work with the Neighbourhood Lead and Neighbourhood Senior Coordinator, member practices and key local operational stakeholders to support coordination of care for patients through a multidisciplinary team meeting approach, stakeholder signposting and supporting the front door across the whole South West footprint.

This will involve the following:

Act as a liaison and link between members of the Neighbourhood to enable co-ordinated patient care, and facilitate improved outcomes for service users and patients in line with their identifiedneeds by supporting the links between health, social and third sector providers.

To support efficient use of services across all organisations and that ensure care is person- centred

To be a central point of navigation, and resource knowledge point, where local care navigators and operational teams have identified the need for greater co-ordination/support.

Development of Neighbourhood network

Public engagement events

Support delivery of projects that are identified by the Neighbourhood, working closely with the Practice Care Coordinators.

Support delivery of projects identified by the locality member practices which are part of the General Practice Forward View (GPFV) and the Lincolnshire Sustainability and Transformational Plan (STP)

Person Specification
Qualifications
  • NVQ Level 3 in Health and Social Care or equivalent qualification / experience.
  • Experience in use of databases
  • Understanding of service user confidentiality Advanced understanding of health and social care processes

  • Excellent communication skills both written and verbal Able to work as a team
  • Able to write reports
