Overview

There is an exciting opportunity for a passionate, experienced cancer nurse to join the Macmillan Lung Cancer Nurse Specialist service, providing high quality patient centred care to those living with lung cancer. The role involves undertaking a comprehensive Holistic Needs Assessment and care planning at key stages during the illness experience, acting as the patient\'s main point of contact coordinating their pathway from the point that lung cancer is suspected. The post holder will have the opportunity to provide specialist support during outpatient consultations, review inpatients and respond to alerts when lung cancer patients are admitted as an emergency, demonstrating the ability to plan and implement programs of care in response to presenting symptoms. We are part of the University Hospitals of Northamptonshire and operate within a trusted, patient‑centred approach across the local NHS foundation trust.

Responsibilities
  • Work as a core member of the lung cancer multidisciplinary team, providing high quality patient centred care from the point of suspicion of the disease.
  • Act as the main point of contact for a caseload of patients, providing specialist information, help and support, and liaising with MDT, primary and secondary care to ensure a seamless service for patients and their families.
  • Support the Macmillan Lung Cancer Clinical Nurse Specialists in delivering an innovative, high‑quality service to those living with lung cancer.
  • Implement key elements of Personalised Care, offering Holistic Needs Assessments and care planning at key stages in the pathway in line with national directives.
  • Provide specialist information during outpatient consultations and hospital reviews, and respond to changes in clinical status to adapt care plans accordingly.

Job description

To undertake a Holistic Needs Assessment at key stages in the pathway using a recognised tool, ensuring the patient\'s physical, emotional, social, financial and spiritual needs are identified.

To develop and implement plans of care through supportive conversations, reviewing and adapting them in response to the changing clinical situation.

To attend and contribute to the weekly specialist multidisciplinary team meeting and coordinate actions from the MDT.

To review patients admitted as an emergency and agree a management plan with the admitting team.

To provide advice on symptom management and refer complex needs to the Macmillan Specialist Palliative Care Team.

To record outcomes of care in hospital notes and on the Somerset Cancer Database, and demonstrate enhanced practice in nursing care.

To provide Level 2 psychological support for patients/carers and refer complex needs to the Macmillan Clinical Psychologist.

To lead on patient and carer communication, coordinating the patient\'s pathway and acting as the main point of contact as needed.

To ensure patients receive specialist verbal and written information at appropriate points in their illness experience, and to be acquainted with local and regional care pathways for referrals and advice.

To identify educational needs of those living with lung cancer and demonstrate knowledge of clinical trials relevant to lung cancer.

To identify the patient\'s Preferred Place of Care and, where possible, discharge to their place of choice; identify patients suitable for the AMBER Care Bundle; and support patients and carers during significant consultations as their advocate.

To promote self‑management and pathways to Macmillan resources and services.

To work in line with the NMC Code of Professional Conduct and promote collaborative working relationships across the cancer multidisciplinary team.
